You are responsible to ensure in-network or out-of-network status as well as plan coverage for any services provided.
Patients are asked to pay their insurance co-payment at the time of their visit. We accept Visa, MasterCard, American Express and Discover as well as personal checks or cash. Any outstanding balance is due within 30 days.
