Medical Records
To request your medical records you can either use your online patient portal (MyChart) or submit your request in writing by mail or fax.
Requests for your TMCOne medical records can be made through TMC Health Information Management. You will need to print out and sign a Request for Medical Records. You can fax the completed and signed form to (520) 324-1590, or scan and email to tmc.medicalrecordsrequest@tmcaz.com.
